powered-by="xiumi.us">孙绪峰，中共党员，1976年5月出生，丰县单楼卫生院高血压门诊医生。他扎根基层二十余载，凭借一名乡村“小医生”的情怀，践行着守护百姓健康的“大梦想”。他刻苦钻研，积极探索医防融合，不断丰富基层首诊签约内涵，挤出时间下村开展健康讲座；自费购药对家庭困难患者精准医疗帮扶，主动开展入户随访。作为百姓健康的守门人，他用最质朴的情怀诠释“医者仁心”的大爱，创立了“六个一”工作模式，秉承“签一位患者，做永远朋友”的工作理念，带领团队解决很多患者“小病拖，大病扛”的问题。</p><section powered-by="xiumi.us">郭庆超，1972年12出生，丰县梁寨镇乡村医生。郭庆超在多年的基层经验中探索“家庭医生签约服务7.3.1”模式，带给老人们儿女般的关怀。“家庭医生7.3.1”模式，就是由家庭医生按照计划每周、每季度、每年对所有辖区内的老年人进行健康随访、评估，开展用药指导和不能研判治疗病情的老人及时联系家人转诊到上级医院就诊。“7.3.1”即每7天对行动不便的老年人进行一次电话访问，每3个月上门对患有慢性病的老年人进行一次巡诊，每1年宣传动员老年人参加1次健康体检，做1次全面评估，做到疾病高危因素的早发现、早干预、早治疗。</p><section
